Just had someone who was not getting correct subaddresses in GUI - but further investigation showed they confused subaddresses with accounts - and that seems to be a legit confusion in the GUI atm

OK, looks like a real error then. There's a known rare-ish bug which can cause db corruption, you might have hit that, though the usual symptoms are different.
